Safety Wars · James Poesl

April 9, 2024. Human and Organizational Performance

April 10, 2026·59 min·5 clips
A petrochemical company's bonus system for managers directly incentivized blaming workers for accidents to protect their own pay.
Changing a rule changes the system. The host starts with familiar work, the kind people can almost do on autopilot, then asks what happens when a new procedure forces heavier decision making. That is where the risk shows up. He separates routine performance, rule following, and knowledge based work, with more room for error at each step. First tries get rough. Borrowed knowledge from another task can help, or it can set up the next mistake. Right? That is the point he keeps pressing. The worker is not the easy villain. The setup, the rule, and the reaction after failure carry much of the pressure. Change the rule, change the risk. He keeps coming back to one uncomfortable question: are you putting someone into a higher error mode? Then he adds the follow up. If failure happens, how will you respond? This is not polished consulting talk. It is a blunt monologue about error, consequence, and accountability. The math is not flattering. Rule based decisions carry far more error than routine work, and knowledge mode can go wrong about half the time. So slow down before rewriting a procedure. Ask what will probably fail, how bad the consequence could be, and whether recovery is possible. A changed procedure is not just paperwork. It tests the person doing the work and the system around them.

As heard by us

A blunt safety piece on autopilot, rule based work, and the cost of changing how people fail.

The episode treats safety as a human problem first. It moves from autopilot to rule based decisions to knowledge based mistakes, then circles back to the same hard point: changing the rules changes the system.
